Preventive Lifestyle Assistance Network (PLAN), LLC is a Harlem-based, M/WBE-certified consultancy at the intersection of public health and the built environment. Our mission is to build healthier, more equitable communities by applying evidence-based strategies to the spaces where people live, work, and learn.
Our foundation in public health is deep and personal. PLAN was launched in 2019 through a direct partnership with the Centers for Disease Control and Prevention (CDC) as a certified provider of its National Diabetes Prevention Program (DPP). Working from our base at the historic Ephesus SDA Church in Harlem, we provided frontline, evidence-based health coaching to our community members both in-person and virtually, empowering them to take control of their well-being.
Today, our mission has expanded from the health of the individual to the health of the entire built environment. We are proud to once again find ourselves partnering with the CDC, alongside the General Services Administration (GSA) and the Center for Active Design (CfAD), as certified Fitwel Ambassadors. This evolution allows us to apply the same rigor and evidence-based approach to buildings that we first applied to community health.

The Fitwel Champion Provider Program is available to companies that want to demonstrate their commitment to healthy buildings and empower their staff and clients to promote and implement Fitwel at scale. The program provides companies access to the highest level of support Fitwel offers and resources to communicate your partnership with clients and customers. Champion Provider companies must register 10 of their own assets or their client's assets and pay for certification of at least 10 or more of those assets over a 12-month calendar year term.
